Job Overview:

The Assistant will work with the professional staff of Care Management in performing discharge planning functions. They provide administrative support and key operational functions toward the goals and objectives for the department. The assistant meets with inpatients to provide information on hospital discharge services and community resources; educates patients and their families on Medicare and Medicaid requirements, including the delivery of Important Message to Medicare Beneficiaries. They will also provide patients with information on Advanced Directives.

Qualifications:

Required:

  • High school diploma
  • Word and Excel knowledge and experience
  • Good oral and written communication skills
  • Proficient typing skills, intermediate to advanced Word, Excel and Access knowledge and intermediate PowerPoint knowledge, excellent oral and written communication skills
  • Independent decision making skills and comprehensive knowledge of managed care concepts, related terminology and data systems
  • Excellent verbal communication and customer service skills
  • Strong communication skills both written and verbal
  • Ability to assist with entire scope of discharge planning (keep statistics, maintain service delivery and follow up with patients and families)
  • Good follow up skills and ability to work independently

Preferred:

  • Bachelor s degree
  • Understanding of hospital policies and procedures and structure of medical center departments
  • Understanding of contractual terms and their implications

Scheduling Requirements:

  • Day Shift- 8:30am to 5pm
  • Monday through Friday
  • Full-Time

Essential Functions:

  • Provides administrative support to Director and Manager
  • Collaborates with Social Work and Case managers to assist in patients discharge planning
  • Assists in making referrals for discharge planning to subacute rehabilitation facilities and visiting nurse agencies
  • Assists in arranging patient transport for patients returning home or to skilled nursing facilities
  • Assists in photocopying patients medical record and compiling necessary documentation as needed for discharge planning
  • Educates patients and their families on Medicare and Medicaid requirements, including the delivery of Important Message to Medicare Beneficiaries
  • Provides information to patients about Advanced Directives
  • Meets with patients and family members to provide information on hospital discharge services and community resources
  • Collaborates with Social Workers to provide patient referrals to public or private agencies for assistance
  • Provides phone coverage for Care Management
  • Provide administrative support to Case Managers and Social Workers
