This role offers the autonomy of independent practice, while working within a collaborative practice model to provide primary care services for frail and elderly patients in long-term care and home care settings. Working with a primary care physician, you would form a team with the nursing-home staff, patients and families to help maintain the health and well being of enrollees. Qualifications
Long-term care or home care experience is helpful, but not required. Prior nurse practitioner experience is not necessary. For Allentown location, experience with ventilators is needed.
We offer a supportive, mentoring environment for new graduates as well as those without geriatric experience. National certification in adult, family or geriatric nursing is required. The desired candidate will be computer savvy, with an outgoing personality and excellent communication skills.
